> A $25.00 nonrefundable deposit fee is due at the time of booking to ensure your desired date.

> Full Payments are due 2 Days prior to your appointment date.

> All appointments must be scheduled at a minimum of 1 week in advance.

> Outside of Oshkosh, there is a TRAVEL FEE of 0.30 per mile

> Payments can be made in Cash or Card. *Card purchases will be sent a pay link*

> Consent Form must be completed for anyone pictured
